He didn`t have his foot stuck, sometimes people will try to jack your car or pull you out and if you time it correctly and take off, they will hang on to the door while you drive. It`s the Euphoria or Endorphin engine or whatever it`s called.

No cheats were used as far as I could tell, the swingset glitch is the most famous glitch in the game, it launches you in a car, i`ve done it many times. The sound effect was from Jurassic park with the hot dog stand, the stand just glitched into the ground.

with a game as huge as that theres no surprise a few glitches will be found, and heck, glitches are awesome.
