Apparently, this user was disguised as an enemy "demo-man" and was being healed by the enemy team medic. As the medic reads his spray he is backstabbed, killing him instantly.

Spies rules, watch out for robusto

Spies in the game can disguise themselves as enemy teammates, and even get healed by the enemy medics. He distracted the enemy medic with a spray on the wall that said "FYI i`m a spy", so the medic looked at it, and before he got it, he was backstabbed.

Spies in Team Fortress 2 can disguise as the enemy team and get enemy medics to heal them when disguised. This spy says to the medic "thanks mate" (the demoman in team fortress 2 is a black Scottish man) Then he puts a tag on the way saying FYI, I`m a spy. Then proceeds to backstab the medic who goes to read it resulting in a one hit KO.
