That "limb" is his right arm. He is reaching down with it to grab the babies feet. It was just a very harsh mistake. That is, if it wasn't photoshopped, and I don't think it was. Very hard to tell with signs though.

I think it might have been...but very limitedly so...like the right arm being thinned out, and lowered...it might have been added entirely, seeing as a lot of signs like these are usually pretty flat (so profile views show only one arm, just like the legs of that guy in the picture, you only see one)
