It looked kinda fake to me. At the end when it shows the spike coming out of his shoulder, his shirt isn't torn. Also looks to be behind him.
Probably some educational video. Gave me cold chills on any note.

come on now guys. look at the camera work. it's too professional. If I were documentary film maker and saw this, I wouldn't have a bunch of different angles when I filmed it. It's staged.

Although, that is how you would do it. They try to miss all the important internal organs to maximize the longevity of the pain the impaled person will suffer.

I learned something useful in history class after all...
